Community Partnerships
Physician Availability
Nurse Practitioner rounds daily M-F. The NP reviews labs, x-rays, visits residents in real time to provide medical care as needed, visits all new admissions within 24 hours and reports findings to the Medical Director.
Medical Director rounds 2 times per week. The MD rounds to all residents needing care each week including all new admissions within 5 days of admit. The Medical Director reviews all nurse notes and meets with the Nurse Practitioner for resident updates and care planning.
Internal Medicine Physicians round weekly. These physicians focus on the treatment and management of chronic diseases and provide comprehensive and routine care to residents as needed.
Wound Care Physician rounds weekly. Our Wound Care physician partnership enhances our wound care program by providing regular treatment and oversight for resident wound needs.
Cardio Renal Physician rounds weekly. Our Cardio Renal physician partnership strengthens our cardiovascular and renal care program by providing regular monitoring, treatment, and expert oversight for residents’ heart and kidney health.
Physiatrist rounds regularly. Our physiatrist partnership enhances rehabilitation care by providing expert oversight, personalized treatment plans, and support for residents’ physical recovery and functional goals.
In-house lab services and X-rays available daily for convenient, timely care.
UHC Medical Group partners with Renton Health and Rehabilitation to offer weekly physician rounds for Optum residents, ensuring consistent, personalized care.

These specialists come to Renton Health and Rehabilitation and provide direct care in our residents’ home ensuring a comfortable location and ease of access to care.
Renton Health also offers Hospice Services with the provider of choice for residents. Our Hospice teams provide patient-centered care with a variety of service options, including music therapy, massage therapy, chaplain services, end-stage dementia support, and sensory comfort items such as weighted teddy bears or stress balls. In addition, our hospice partners provide palliative and respite care to support residents’ comfort, quality of life, and the needs of their families.
In-house transportation and partnerships with HopeLink and Access provide residents with easy access to appointments and community events.
